Pasaportun aslı : 10 yıldan eski olmamalı, içerisinde 2 adet boş vize sayfası bulunmalı ve planlanan dönüş tarihinden itibaren 1 yıl daha fazladan
geçerlilik süresi olmalıdır. Yeni pasaportun ve varsa daha önceden alınmış eski pasaportların bütün işlenmiş sayfalarının fotokopileri (kimlik
bilgileri, geçerlilik süreleri, varsa vizeli ve mühürlü sayfalar, vb.)
Valid Passport, The passport should still be valid 1 year further from application date; photocopy of all pages of the passport contain stamps and
visas). Passport must have sufficient space for a visa to be inserted, at least two empty pages are required. If your passport does not comply
with the above your application will be returned to you.
-2 adet Dikkatlice doldurulmuş ve imzalanmış başvuru formu (formun Ingilizce, Fransızca, Almanca veya Flemenkçe doldurulması gerekecektir)
-Two fully completed and signed visa application forms + pictures(Please complete it in French, Dutch, German or English).
-Sonuç belgesi
-Signed declaration on desired language as per visa application submission location.
- Standartlara uygun (yüz görünür), yeni çekilmiş, beyaz fonlu iki adet vesikalık fotoğraf (35X45 abatlarında). Fotoğrafın arkasına isim soyisim
yazılması gerekiyor.
-On extra recent photograph (not older than 6 months) 35x45 in colour, preferably biometric type with white background. Please write your
name and last name on the back of the photographs you submit along with your application. Total 2 Photos
-Belçikadaki kurumdan finansal söz ve Belçikadaki belediyeden onay ve Kabul metubu. (orjinal ve +2 fotokopisi)
-A pledge of financial support signed by the establishment in Belgium you have received an acceptance from and approved by the municipal
authorities in Belgium (original and two photocopy)
-21 yaş üstü öğrenciler için 6 aydan daha eski olmayan, Ankara Adli Sicil Müdürlüğü tarafından Belçika resmi dillerinden biriyle yazılmış adli sicil
kaydı aslı ve fotokopisi. Belge Dışişleri Bakanlığı veya adli mahkeme tarafından onaylanmış olmalı. (apostille) aslı ve +2 fotokopisi
-For students over 21 years of age: A certificate of good conduct dating from no more than six months from the Judicial Records Directorate in
Ankara + translation + 2 photocopy. It must be certified by the Foreign Ministry or the Judicial Court (apostille)
-Belçika Elçiliği veya Konsolosluğu tarafından belirlenen bir doktordan alınmış Sağlık Belgesi, 3 aydan daha eski olmamalı aslı ve +2fotokopisi
-A medical certificate, obtained from the doctor designated by the Embassy of Belgium or Consulate General of Belgium (recent, not older than
3 months) and 2 photocopy
-Reşit olmayanlar için (18 yaş altı): Yasal velisini gösterir belgeler (Kimlik cüzdanı fotokopisi, tam tekmil vukuatlı nufüs kayıt örneği vb.)
Ebeveynleriyle birlikte seyahat etmiyorsa, ebeveynlerinin noter tasdikli izin belgesi (Muvaffakatname), apostille
-For applicants under 18, a notarized permission letter from the parents or guardian in Turkey with the statement: “I allow my child to study
abroad “, its translation in one of the official languages of Belgium or in English. This document must be approved by the Foreign Ministry or the
Judicial Court. (apostille). + 2 photocopy
-Bütün evrakların +2 fotokopisi ve aynı sıralamada sunmanız gerekmekte
-Photocopy of the complete file (the documents listed above must be organized in the mentioned order)
-Vize ve servis ücreti (nakit ve türk lirası olarak başvuru ofisimizde ödenecektir)
-Service fee amount (to be paid in cash in Turkish Liras)
