1. Toplama merkezi/karantina istasyonunda bulundukları tüm zaman boyunca, SBV taşıyıcı haşerelere
karşı koruma altında olduğunu / were subjected to measures aimed to protect them against insect
vectors, which can transmit the SBV, during their entire stay at the collection centre/quarantine

veterinary health certificate and must be attached to it in an indivisible manner / Questo allegato fa parte del certificato
veterinario e deve essere unito al suddetto in modo indivisibile.
(2) Ekli listede söz konusu bilgiler yer almalı ve belgeyi düzenleyen resmi veterinerin imzası ve mühürü belgede yer
almalıdır. Bilgilerin yer aldığı belge ek beyana ayrılmayacak şekilde eklenmelidir / The enclosed list must include these data
and be signed and officially sealed by the certifying official veterinarian and must be inseparably attached to the additional
declaration / L’elenco allegato deve includere questi dati ed essere firmata e timbrata ufficialmente dal veterinario ufficiale
incaricato della certificazione e deve essere unita alla dichiarazione aggiuntiva.
(3) Mühür ve imzanın rengi sertifikanın baskı renginden farklı olmalıdır / The colour of the official seal and the signature
must be different from the printing colour of the certificate / Il colore del timbro ufficiale e della firma deve essere diverso da
quello del testo stampato del certificato.
